METOPROLOL (Lopressor®, Toprol XL™) belongs to a group of medicines called beta-blockers. Beta-blockers reduce the workload on the heart and help it to beat more regularly. Metoprolol controls, but does not cure, high blood pressure (hypertension). High blood pressure may not make you feel sick, but it can lead to serious heart problems. Toprol-XL is the most widely prescribed branded beta-blocker in the U.S. The new low-dose, scored 25-mg tablet is in addition to the 50-mg, 100-mg and 200-mg dividable tablets. The recommended starting dose of Toprol-XL in heart failure is 25-mg once-daily for two weeks in patients with NYHA class II heart failure and 12.5-mg once-daily in patients with more severe heart failure. If this dosage is tolerated, it may be doubled every two weeks to the highest dosage level tolerated by the patient or up to 200-mg.

February 6, 2001 - The FDA has approved using Toprol-XL (metoprolol extended release) for heart failure. The drug was already approved to treat high blood pressure and angina. The FDA's decision was based on trials showing that Toprol-XL reduces mortality rates by 34% in CHF patients.

Toprol XL is a beta blocker which is sometimes effective in the prevention of migraine. It is a cardio-selective beta blocker which can be used for some patients who have co-existing asthma. This compound like other beta blockers is devoid of instrinsic sympathomimetic activity, and theyhave proven comparable in their effectiveness in prevention of migraine headaches.

If selected for treatment, the recommended starting dose of TOPROL-XL is 1.0 mg/kg once daily ... the maximum initial dose should not exceed 50 mg once daily. The minimum available dose is one half of the 25 mg TOPROL-XL tablet. Dosage should be adjusted according to blood pressure response. Doses above 2.0 mg/kg (or in excess of 200 mg) once daily have not been studied in pediatric patients.
