Congenital arteriovenous shunts are usually asymptomatic, while the acquired type often are associated with pulsatile tinnitus. Common causes are head trauma or surgery. Glomus tumor is a vascular neoplasm arising from the paraganglia around the carotid bifurcation, the jugular bulb, or the tympanic arteries. These tumors usually cause a loud pulsating tinnitus.20

Pulsatile tinnitus (tinnitus that beats with your pulse) can be caused by aneurysms, increased pressure in the head (hydrocephalus), and hardening of the arteries. Anything that increases blood flow or turbulence such as hyperthyroidism, low blood viscosity (for example, anemia), or tortuous blood vessels may cause pulsatile tinnitus. Vitamin B12 deficiency is common in tinnitus patients.
About half the people with tinnitus hear their tinnitus in both ears at the same time. About 10% hear it in their left ear, and another 10% hear it in their right ear. For the remainder, they just hear their tinnitus inside their head somewhere.
Other causes of tinnitus may be as benign as a small plug of wax in the ear that is easily removed. Tinnitus may ... occur as a result of head and neck injury, stiffening of the bones in the middle-ear or as a side-effect of prescription drugs.
The mechanisms of subjective tinnitus are often obscure. While it is not surprising that direct trauma to the inner ear can cause tinnitus, other apparent causes (e.g., temporomandibular joint disorder (TMJ) and dental disorders) are difficult to explain. Recent research has proposed that there are two distinct categories of subjective tinnitus: otic tinnitus, caused by disorders of the inner ear or the acoustic nerve, and somatic tinnitus, caused by disorders outside the ear and nerve but still within the head or neck. It is further hypothesized that somatic tinnitus may be due to "central crosstalk" within the brain, as certain head and neck nerves enter the brain near regions known to be involved in hearing.
