Strep throat is caused by the bacterium Streptococcus pyogenes.[5][6][7][8] It is spread by direct, close contact with an infected person via respiratory droplets. In one study, 114 patients with positive throat cultures and a presence of group A beta-hemolytic streptococci (GAS) were studied to see transmission within their families through pillow cases, toys, and patients tooth brush. Of those 114 patients studied 50 patients received hygiene instructions to follow and 64 were not given hygiene instructions. The percent of culture-verified recurrences was 35%.[9] Casual contact can result in transmission like the study that was done on transmission in households by two Norwegian health centers. 127 patients with positive throat cultures and their families were studied. Four weeks after the beginning of the study, other members of the household were asked if they had any positive throat culture since the beginning of the study; In 30 households there was one more case of infection, in 22 households 2 more cases, 6 households with 3 cases, and 2 households with 4 cases.[10] Rarely, contaminated food, especially milk and milk products, can result in outbreaks.
Strep throat infections are spread person-to-person. Humans are the natural reservoir of this bacteria. The nose and back of the throat are the main sources of carriage of this bacteria. The skin and feces are potential sites. Aerosolized upper respiratory mucus serves as the primary source of the strep germ spreading to others. Direct contact with infected nose and throat tissues (by kissing) is of less importance as is contact with contaminated objects, such as toothbrushes.

There is a characteristic rash that sometimes accompanies strep throat. This appears as fine, red pimples all over the trunk (chest, abdomen and back) that feel rough, almost like sandpaper and may look like a sunburn. This rash, with strep throat, is called Scarlet Fever. Do not worry; this is not serious like it used to be. It is simply the body reacting to a toxin produced by the strep. It does not mean the infection is more serious than strep throat without a rash, and it goes away with treatment.
