Stephanie began a relationship with wrestler Test which led to a heated feud between Test and her older brother, Shane McMahon, who viewed Test as not being good enough for Stephanie. After Test defeated Shane at SummerSlam (1999) in a "love her or leave her" Greenwich Streetfight, Shane stopped objecting and went out of his way for his sister and for Test, proving to them he was on their side. Stephanie and Test went on to team together in a match on September 20, 1999, with the couple defeating Jeff Jarrett and Debra. The couple were eventually engaged, but their original wedding date had to be postponed after The British Bulldog accidentally hit Stephanie with a thrown trash can, giving her a concussion and amnesia. A later date was set, but during the in-ring ceremony, Triple H showed a video which revealed that he had drugged Stephanie and taken her to Las Vegas, Nevada where they were married in a drive-through ceremony. This effectively ended the relationship between her and Test.

Stephanie McMahon, who was married to Triple H at the time, got a shot at Jacquelyn's Women's Championship, despite having extremely limited experience in the ring. But although Jacquelyn had much more experience, Stephanie had D-Generation X by her side. Stephanie goaded Jacquelyn into chasing her around the ring, which allowed X-Pac to trip her setting up a DDT by Tori. Stephanie got her back in the ring and covered her for the pin and her first Women's Championship.

Stephanie McMahon-Helmsley then headed to the ring, and the former Women's Champion was not in a good mood. Stephanie compared Trish Stratus to a pile of garbage, and she demanded that Trish come to the ring. Trish said that Stephanie had it all wrong, and that Mr. McMahon was just a mentor to her. Trish said that she and Stephanie were the same, and that she loved the business. Trish said she would bend over backwards for the World Wrestling Federation. Stephanie told Trish never to compare herself to her. Stephanie declared that she was the dominant female in the World Wrestling Federation, and she would prove it -- the easy way or the hard way.

By The Ringmaster Stephanie McMahon was reportedly furious after the segment between Big Show & Floyd Mayweather went down on RAW. Steph apparently chewed out Bruce Pritchard, who was the person in charge of directing Floyd on RAW.

Vince introduced the General Manager, Stephanie McMahon to the ring. Vince didn't let her music play because he didn't seem too pleased with her. Vince then asked Stephanie how she got him into this mess. Tell him how and why she signed so called Mr. America to a contract. Stephanie said she did what she thought was right for the fans of Smackdown!. She did what she thought was right for business.

The internal disputes between the McMahons led to Shane siding with his mother against Stephanie and Vince. Linda was in a comatose state due to the stress of being asked for a divorce by Vince, and Vince took the opportunity to have a public affair with Trish Stratus. McMahon briefly feuded with Stratus, defeating her at No Way Out 2001, but eventually both settled their differences and sided with Vince. Over the next few weeks, Vince made it clear that he favored McMahon over Stratus, even allowing McMahon to cover Stratus with mud using a mop in the middle of the ring. At WrestleMania X-Seven, Shane defeated Vince in a street fight. During the course of the match, Stratus slapped Vince and chased McMahon from ringside, apparently upset with Vince's constant misogynistic treatment of her.
