You can apply for Voluntary Insurance at any office of Social Security if you have been employed, have at least 150 contributions paid on your behalf and are under the age of 65. You need to apply within 26 weeks of your last day of employment to be eligible.

The Social Security Regulations include the "listings" which describe how severe a disease, condition or impairment must be for the Social Security Administration to consider it disabling.
