Plain radiography is generally obsolete, but exceptions include its use in confirming air-fluid levels in acute sinusitis and evaluating size and integrity of the paranasal sinuses. Radiographs may still provide a useful adjunct to diagnosis in parts of the world, where sophisticated imaging is not yet available.

Quit smoking Acute sinusitis often resolves on its own without the need for treatment. If treatment does become necessary, decongestants or antibiotics may be prescribed. Chronic sinusitis can be more difficult to treat and may require treatment with several different medications, including antibiotics, decongestants, mucus-thinning medications, and antihistamines or anti-inflammatory corticosteroid nasal sprays (both of which are used to reduce swelling and mucus production).

With the advent of CT, the role of conventional radiography has taken second place and presently has a limited role in the management of sinusitis. There are wide intraobserver differences in the interpretation of plain radiographs, and the rate of false-negative results is high.

The possible benefit of using steroid nasal sprays for treating acute bacterial sinusitis has not been adequately studied. This treatment is not recommended because of its additional costs to the treatment program and because antibiotic treatment alone is highly effective.
