If... the reference to the second arising of the Dark Tower is taken literally rather than taken to mean something else, then the Mouth Of Sauron has been in the service of his master for 68 years. If this service began as a youth (perhaps a child taken by Sauron as a promising student of sorcery from a "renegade" family, for example) who was subsequently cowed, bewitched and indoctrinated by his new master, then The Mouth of Sauron could quite feasibly have no memory of his birth name. Sauron enjoyed nurturing very powerful beings from their infancy, as his personal rearing of the Nazgûls' 'Fell Beasts' indicates.

Sauron did not know what to make of the presence of a Hobbit in his land. It still had not entered his thoughts that anyone would try to destroy the Ring, and besides, the Hobbit had not had the Ring with him. Although the Hobbit was still at large, Sauron thought that he might be able to make use of his possessions. They were given to Sauron's emissary, the Mouth of Sauron, who was told to show them to Gandalf especially.

The Mouth of Sauron was ... known as the Lieutenant of Barad-dûr, he had served Sauron for much of his life so had forgotten his own name. He was a man of great stature, he was potentially the equal of other Dúnedain, but only had fallen into darkness.
