In the eye, the lacrimal gland is a common area of manifestation for sarcoidosis. produce tears to moisten and protect the eyes. It's not unusual for sarcoid patients to have uncontrolled tearing of the eyes, dry eyes or blurry vision.

Lymphadenopathy is very common in sarcoidosis. Intrathoracic nodes are enlarged in 75 to 90% of all patients; usually this involves the hilar nodes bilaterally, but some may only have unilateral enlargement,the paratracheal nodes are ... commonly involved. Peripheral lymphadenopathy is very common, particularly the cervical, axillary, epitrochlear, and inguinal nodes. The nodes are non-tender, non-adherent, with a firm, rubbery texture. Unlike nodes in tuberculosis, they do not ulcerate.

For pulmonary sarcoidosis, the initiation dosage is 20 to 40 mg per day of prednisone or its equivalent for one to three months. Every-other-day dosing ... may be considered. In patients who respond, the prednisone dose should be tapered to 5 to 10 mg per day or every other day for a minimum of 12 months.

Lung transplantation is a viable option for patients with stage IV sarcoidosis. Transplantation in such patients should be strongly considered when the forced vital capacityfalls below 50% predicted and/or the forced expiratory volume in 1 second falls below 40% predicted.

What happens in later stages of sarcoidosis is the lymph nodes shrink down; they stop doing their part. This may be because they become too damaged to function. It's not unusual for lymph nodes to be calcified in sarcoidosis patients.

About 50% of sarcoidosis patients improve spontaneously. The disease is fatal in less than 5% of patients. In between the two extremes, patients have mild to severe sarcoidosis, with various degrees of impairment or none at all.
