Answer: Coumadin and Rocephin are not examples of drugs that require intensive monitoring for [T]oxicity. Coumadin monitoring is not intensive and the drug is not toxic – the monitoring is needed to keep the dose in the effective range but usually is only done monthly after initial implementation. Examples of drugs categorized as high risk, are most chemotherapy drugs and certain cardiac drugs with narrow therapeutic ranges such as Amiodarone. If you look in a drug book, you will see that Amiodarone is listed as “toxic”, but drugs like Coumadin and Rocephin are not.

Due to its effectiveness against a broad range of bacteria, the prescription antibiotic Rocephin is used to treat many common bacterial infections, and is popular as a substitute for penicillin and for preventing infection during surgery. Rocephin works by keeping bacteria from making and maintaining their cell walls. A few of the infections commonly treated with Rocephin include blood infections, middle ear infections (otitis media), and irritation of the prostate (prostatitis).

IM Ceftriaxone (Rocephin) has received attention as an alternative treatment for AOM. Ceftriaxone (50 mg/kg/day, given as a single dose IM) was shown to be an effective alternative for treatment of AOM and is considered one of several "second line" choices to amoxicillin. Barnett, et.al. (click here for the electronic abstract) have recently published data showing a single IM dose of ceftriaxone has very similar clinical efficacy to 10 days of oral Trimethoprim-Sulfamethoxazole. Potential exists for this route of administration in select patients that may be vomiting, have a history of poor compliance, are difficult to follow up, or have failed initial antibiotic therapy. Roche is informing health care providers of revised prescribing information for Rocephin (ceftriaxone sodium) injection based on new reports describing the potential risk of its concomitant use of with calcium or calcium-containing solutions or products. Fatal cases of calcium-ceftriaxone precipitates in the lungs and kidneys in both term and premature neonates have been reported. Hyperbilirubinemic neonates, especially premature neonates, should not be treated with Rocephin, and the drug must not be mixed or administered simultaneously with calcium-containing solutions or products. Additionally, calcium-containing solutions or products must not be administered within 48 hours of the last ceftriaxone administration.

Rocephin is given as an injection into a muscle or a vein. Your doctor, nurse, or other healthcare provider will give you this injection. You may be given instructions on how to use your injections at home. Do not use this medication at home if you do not fully understand how to give the injection and properly dispose of needles, syringes, and other items used in giving the medicine.

Before instituting treatment with Rocephin, appropriate specimens should be obtained for isolation of the causative organism and for determination of its susceptibility to the drug. Therapy may be instituted prior to obtaining results of susceptibility testing.
