Risperidone is an atypical antipsychotic agent for two reasons. First, it is chemically unrelated to the older antipsychotic drugs. Second, unlike older antipsychotic drugs that primarily inhibit the actions of dopamine, a chemical in the brain, risperidone may ... have some action against another brain chemical, serotonin. The proper level of both dopamine and serotonin are influential in maintaining mental well-being.
The mechanism of action of Risperidone , as with other antipsychotic drugs, is unknown. However, it has been proposed that this drug's antipsychotic activity is mediated through a combination of dopamine type 2 (D2) and serotonin type 2 (5HT2) antagonism. Antagonism at receptors other than D2 and 5HT2 may explain some of the other effects of Risperidone.

Risperidone is metabolized to 9-hydroxyrisperidone by cytochrome P450IID6, an enzyme that is polymorphic in the population and that can be inhibited by a variety of psychotropic and other drugs (See ACTIONS/CLINICAL PHARMACOLOGY). Drug interactions that reduce the metabolism of risperidone to 9-hydroxyrisperidone would increase the plasma concentrations of risperidone and lower the concentrations of 9-hydroxyrisperidone. Analysis of clinical studies involving a modest number of poor metabolizers (n is approximately equal to 70) does not suggest that poor and extensive metabolizers have different rates of adverse effects. No comparison of effectiveness in the two groups has been made. In vitro studies showed that drugs metabolized by other P450 isozymes, including 1A1, 1A2, IIC9, MP, and IIIA4, are only weak inhibitors of risperidone metabolism.
