At WrestleMania 22, Rey Mysterio showed the world that dreams really do come true. Coming into the event, no one thought he could walk out of Chicago as the World Heavyweight Champion. Mysterio was no stranger to playing the role of the underdog, so he was right at home when he faced Kurt Angle and Randy Orton in a Triple Threat Match for the World Heavyweight Championship. Rey was overpowered early in the match, but he showed persistence and fought back. Mysterio utilized his unique blend of quickness and agility as he soared through the air hitting the Legend Killer with a West Coast Pop into a pinning combination. Just like that, the undersized Mysterio was the World Champion.

When Rey Mysterio won the 2006 Royal Rumble, the last elimination was Randy Orton. Still upset by this, on the February 3, 2006 episode of Smackdown!, Orton interrupted Mysterio's victory speech and verbally attacked him, saying "there was no way Mysterio could ever beat him in a real wrestling match". So Orton wanted to challenge Rey for his Wrestlemania 22 title shot. As Mysterio looked upwards, Orton continued by saying that "Eddie Guerrero is not in heaven, he is 'down there'-- IN HELL!" Infuriated, Mysterio attacked Orton with a flurry of enraged punches, while security tried to stop him. He lost his tag-team match later that night with Kurt Angle against Orton and Mark Henry, and afterward put his title shot on the line by fighting Orton at No Way Out on February 19, 2006.

On January 29, 2006 Rey Mysterio competed in the Royal Rumble's "Royal Rumble match" where he entered in the number 2 position, lasting over an hour in the ring to finally eliminate Randy Orton and win the match. By winning the Royal Rumble match, Mysterio gained a World Heavyweight title shot at that years WrestleMania which would be the first time he competed for a World Title during a pay-per-view event. Also, Mysterio set a Royal Rumble record by lasting over 62 minutes in the match.

Rey Mysterio stunned the world when he won the 2006 Royal Rumble. At WrestleMania 22, he became the World Heavyweight Championship by beating champion Kurt Angle and Randy Orton. He celebrated his victory with Vicki and Chavo Guerrero. They have since turned on him because he is not a Guerrero and cost him the World Championship.

The challenge took place at No Way Out with Randy Orton defeating Rey Mysterio while illegally using the ring ropes to aid a pin. Having lost the title shot at WrestleMania 22, Rey cried backstage with a few of Guerrero's close family. However on a later edition of Smackdown!, the sympathetic SmackDown! General Manager Theodore Long declared the World Title match would include Mysterio, making the match a Triple Threat that would see Rey face off against both Randy Orton and then-World Heavyweight Champion Kurt Angle. Some questioned Long's decision, namely Orton, who was furious that he would not be the only one challenging for the title at WrestleMania. However, Angle, the champion, didn't seem to care because he had no qualms about defeating the both of them.
