Remicade (Infliximab), an immune-suppressing drug used to treat rheumatoid arthritis and Crohn's disease, is reported to be responsible for 70 reported cases of tuberculosis (TB) among US patients. The cases, at least four of which were fatal, were reported to the Food and Drug Administration between 1998 and May of this year. Researchers who reviewed the cases say that the immune-system protein infliximab suppresses called tumor necrosis factor alpha (TNF-alpha) appears to be key in controlling TB infection.

��� Remicade, which generated $3 billion in revenues for J&J last year... faces scrutiny over safety concerns. Due to increased risk of infections and cancer, the U.S. Food and Drug Administration has issued two black box warnings for Remicade — its strongest safety warning. Humira, a comparable drug produced by J&J’s competitor Abbott, carries only one black box warning, while Enbrel, produced by Amgem, carries no black box warning.
In interviews with Reuters Health, spokespeople for Enbrel's maker Immunex, and Centocor, the maker of Remicade, emphasized that demyelinating disease is a rare complication of therapy with these two drugs. The companies have known about this complication for over a year, and a warning about it appears on their drug packaging and Web sites.

Over the past decade, Remicade has significantly expanded the Crohn's disease market. Additionally, robust expansion of this market through 2016 will be driven by the rapid uptake of Humira and novel biologics currently in late-stage clinical development, as well as by more aggressive medical practices. Despite high unmet need and significant market growth expected over the next decade, barriers to entry remain high, as evidenced by recent failures and regulatory setbacks experienced by several drugs.
Previous studies evaluating the safety of Humira and Remicade focused specifically on the drugs' ability to cause lymphoma, pneumonia or tuberculosis. The Mayo Clinic study was able to determine a link between use of the rheumatoid arthritis drugs and the development of several other cancer types, including skin cancer, gastrointestinal cancer, breast cancer and lung cancer.
