Your completed Registration Form, including proof of eligibility, proper payment, and signature must be mailed together to the CHSPE Office at the address shown on the Registration Form. If applicable, your completed Accommodations Request Form with the required documentation, clergy letter, and/or letter describing any physical access needs must ... be included. Refer to the CHSPE Administration Schedule for registration deadlines and to determine if you need to include the late registration or emergency registration fee. Refer to "Payment Policies" for information about the payment of fees.

Registration fees for material submitted by mail or in person are payable in cash, check, money order, or credit card (Visa/MasterCard). All fees paid either by check or money order must be payable in U.S. dollars, as well as drawn from a U.S. bank account. Checks and money orders may be payable to WGAW.

Advance registration is recommended for your convenience and to avoid possible delays registering on site in San Diego. An early-bird discount is offered for registrations received by August 15, 2008. Full registration fees are in effect for registrations received between August 15 and September 22. Onsite fees are in effect after September 22. Name badges, tickets and other conference information will be mailed to US and Canadian advance registrants approximately 3 weeks prior to the APHA Annual Meeting. International advance registrants will received general meeting information only -- badges will not be mailed.

The registration fee and the CHP fee are due for the vast majority of vehicles registered for use on the highway. This money is used by the California Highway Patrol (CHP) and the Department of Motor Vehicles (DMV) to offset costs. A Reflectorized License Plate fee is assessed upon initial registration on most vehicles.

An owner or operator of a Utah based fleet of commercial vehicles operating in two or more jurisdictions may apply for an apportioned registration. This registration allows commercial vehicles to comply with registration requirements of more than one jurisdiction and to pay registration fees based on the percentage of operation in those jurisdictions. The prorate percentage for each requested jurisdiction is determined as a fraction, the numerator of which is an amount equal to fleet mileage traveled in that jurisdiction and the denominator is total miles operated by the fleet in all jurisdictions. The total bill is determined by adding the amounts for each requested jurisdiction. For further information, contact the Motor Carrier Services Section, (801) 297-6800.

Cancellation of registration for a Pre-Meeting Workshop is subject to the same policies and dates noted above with the exception that written cancellations must be postmarked or faxed before November 30; the handling fee is $25. Refunds will not be issued for cancellations postmarked or faxed after November 30. If ASHP cancels a workshop, full refunds will be issued to registrants.
