Tacrolimus chemical structure. Prograf undergoes extensive hepatic first pass metabolism. Metabolites are primarily excreted via the bile. Less than 1% of unchanged tacrolimus is seen in the urine after intravenous or oral dosing. There is evidence that Prograf is metabolised in the gastrointestinal tract when given orally.

Tacrolimus (Prograf®) will decrease your body's ability to fight infections. Call your doctor or health care professional if you have a fever, chills, sore throat or other symptoms of a cold or flu. Do not treat these symptoms yourself. Try to avoid being around people who are sick. Do not have any vaccinations without your doctor's approval. Avoid people who have recently received the oral polio vaccine.
As Prograf may alter the metabolism of oral contraceptives, other forms of contraception should be used. It is advised that patients are counselled regarding the risks of becoming pregnant whilst receiving Prograf therapy. In animal studies (rats and rabbits), Prograf has been shown to be teratogenic at doses which ... demonstrated maternal toxicity. Preclinical and human data show that the drug is able to cross the placenta.

Prograf is available for oral administration as capsules (tacrolimus capsules) containing the equivalent of 0.5 mg, 1 mg or 5 mg of anhydrous tacrolimus. Inactive ingredients include lactose, hydroxypropyl methylcellulose, croscarmellose sodium, and magnesium stearate. The 0.5 mg capsule shell contains gelatin, titanium dioxide and ferric oxide, the 1 mg capsule shell contains gelatin and titanium dioxide, and the 5 mg capsule shell contains gelatin, titanium dioxide and ferric oxide.

These occur with oral and IV administration of Prograf and may respond to a reduction in dosing. Diarrhea was sometimes associated with other gastrointestinal complaints such as nausea and vomiting.
