Isolated from mare's urine (PRE[G]nant MARes' urINe), it is manufactured by Wyeth Pharmaceuticals and has been marketed since 1942. On May 5, 1997, a little noticed news item reported that the U.S. Food and Drug Administration (FDA) had rejected an application for a generic version of the drug Premarin. The FDA approving or rejecting a drug is generally a dog bites man story. But not in this case. Indeed, it appears that in making one of the most important decisions in recent years, the FDA sacrificed science and the public health on the altar of political accommodation. Although the FDA and Premarin's manufacturer, Wyeth-Ayerst, would like the American public to believe that this decision was in their best interests, the reality is that it was driven by hoards of lobbyists, fraught with conflicts of interest, and characterized by questionable behind-the-scenes political maneuvering.

Premarin, the brand name for conjugated estrogens manufactured by Wyeth Ayerst Pharmaceuticals is a medication used during hormone replacement therapy for treating menopausal symptoms. The medication is derived from the urine of pregnant mares and contains a number of different estrogens. A July 2002 study conducted by the National Cancer Institute linked Premarin use to an increased risk of ovarian cancer. Side effects of Premarin include sharp chest or leg pain, breast lumps, severe headache, severe stomach pain, abnormal vaginal bleeding and shortness of breath.

Wyeth, the maker of Prempro ... manufactures Premarin, which shares the same estrogen component. In 1975, Premarin became Wyeth's best selling drug. It was also the 5th best seller in the nation. Wyeth did all they could to get the drug approved to treat symptoms of heart disease and osteoporosis. Recent studies of the National Institute of Health clearly show that Wyeth’s efforts in studying the results of Premarin were inaccurate. They found that the Premarin/Provera combnation doesn’t prevent heart disease; it in fact may cause it.

In July of 2003, federal regulators approved a lower-dose version of Premarin based HRT, and it is this lower dose that Wyeth is now recommending (specifically Prempro 0.45 mg/1.5 mg doses). Prempro 0.45 mg/1.5 mg contains 28% less estrogen and 40% less progestin than the current standard dose (Prempro 0.625 mg/2.5 mg). This new focus by Wyeth is cited as one reason for its recent cut in PMU producing contracts.
