Parents of patients infested with pinworm may ask the pharmacist how to prevent a future problem and how to maintain cleanliness during an ongoing episode. Several commonsense steps can be followed to decrease the risk of reinfection. Most require added attention to hygiene.6,7 For instance, the patient should shower every morning to decrease the chance of spreading the parasite to others and of retroinfection. Showering is preferable to bathing, since soaking in the bath with eggs floating in the water could conceivably allow them to enter the genitourinary tract. Patients should be urged to change underwear and sleepwear each day and night. Each period of wear should be followed by thorough washing.

Infection occurs after pinworm eggs (ova) are swallowed. The eggs develop into larvae in the small intestine, then move to the lower intestine. There, the larvae mature within 2 to 6 weeks, and the adult worms mate. After the eggs develop, the adult female worm moves to the rectum and exits through the anus to lay eggs. The eggs are deposited in a sticky, gelatinous substance that sticks to the skin around the anus. From there, eggs can be transferred to fingernails, clothing, bedding, toys, or food.

The tomato pinworm is a small, microlepidopteran moth that is often confused with closely related species which have similar habits. Apparently, much of the damage to tomatoes attributed to the eggplant leafminer (Gnorimoschema glochinella Zeller) in Mexico and California during the early 1920s was actually inflicted by the tomato pinworm (Morrill 1925). It persisted in the literature as the eggplant leafminer until redescribed as a new species (Busck 1928) collected from tomatoes. It was later synonymized with Eucatoptus lycopersicella Walshingham. Capps (1946) provided a key, with descriptions, that defines the species and permits identification of larvae with which it might be confused.

You get pinworms by unknowingly swallowing microscopic pinworm eggs. These eggs pass into your digestive system where they hatch. A couple of weeks later, female pinworms migrate to the area around the anus where they lay their eggs. This triggers itching around the rectum. When you scratch the itchy area, you transfer microscopic pinworm eggs to your fingers. Your contaminated fingers can transmit the parasites to many surfaces, including bedding, clothing and food.

An itchy anus is the most common symptom of a pinworm infestation. It is caused by the adult worm migrating out through the anus to lay eggs. This usually occurs at night, which is when the itching will be most intense. The first signs and symptoms may not appear until one to two months or more after the person swallows the pinworm eggs.
