Phenytoin is a category D drug. Various congenital anomalies have been reported from usage during pregnancy (see Fetal hydantoin syndrome). No scientific data have demonstrated that effect or outcome of acute toxicity is based on sex.

Phenytoin rarely induces other types of inflammatory and disorders of keratinization. Phenytoin has been linked to drug-induced linear IgA bullous dermatosis.[131] It may be linked to the development of sarcoidosis,[132] specifically pulmonary sarcoidosis.[133] Thickening of the heel pad associated with long-term phenytoin therapy has been reported.[134]

The pseudolymphoma syndrome has been described as a hypersensitivity reaction to anticonvulsant drugs especially Phenytoin (Diphenylhydantoin). Its pathogenesis is unknown. It consists of a triad of fever, generalised rash and lymphadenopathy. In some cases, varying degrees of malaise, hepatosplenomegaly, abnormal liver function tests, arthralgia, eosinophilia and blood dyscrasias may be present. [1]
