Ustinov's first acclaimed film role came in 1951 starring as Nero in Mervyn Leroy's Quo Vadis. The role earned him his first Oscar nomination. In the 1960s, he went on to earn two more Oscar nominations for acting - and two wins. He picked up his first supporting actor Oscar in 1961 for his role in Stanley Kubrick's Spartacus. He won the same award in 1965 for starring in the caper classic Topkapi. Ustinov earned one additional Oscar nomination - for his 1968 screenplay Hot Millions.

Ustinov was originally offered the role of Police Inspector Jacques Clouseau in the 1963 film The Pink Panther but he dropped out days before shooting commenced. Upset by this, Blake Edwards successfully sued Ustinov.

Ustinov, der seit Jahren an Diabetes und Ischias litt, war zuletzt auf den Rollstuhl angewiesen. Das hinderte ihn keineswegs, wie der Reporter der FAZ berichtete, sehr agil und beweglich seinen „Auftritt“ vor dem Medienvertreter zu inszenieren. Er starb am 28. März 2004, im Alter von 82 Jahren, in einer Privatklinik in Genolier bei Genf an Herzversagen. Sein Grab ist auf dem Friedhof von Bursins (Kanton Waadt/Schweiz).
