This project in Madagascar formally took shape in February 1998, when Heinz and Alice Hoenecke visited the SALFA headquarters in Antananarivo. Discussion with SALFA officials lead to the formation of a plan to provide histopathology services to the people of Madagascar and to train selected Malagasy physicians in the field of Pathology. A proposal was submitted to the United State Agency for International Development (USAID) for financial support and a grant was subsequently awarded.

France invaded Madagascar in 1883 in what became known as the first Franco-Hova War seeking to restore property that had been confiscated from French citizens. (Hova is one of three Merina classes: andriana - aristocracy, hova - common people, andevo - slaves. The term hova was wrongly used by the French to mean Merina.) At the wars end, Madagascar ceded Antsiranana (Diego Suarez) on the northern coast to France and paid 560,000 gold stripers francs to the heirs of Joseph-François Lambert. In 1890 the British accepted the full formal imposition of a French protectorate over Madagascar that had been imposed in a milder form in 1885, in return for eventual control over Zanzibar (now part of Tanzania) and as part of an overall definition of spheres of influence in the area.
In 1937, Poland sent a commission to Madagascar to determine the feasibility of forcing Jews to emigrate there. Members of the commission had very different conclusions. The leader of the commission, Major Mieczyslaw Lepecki, believed that it would be possible to settle 40,000 to 60,000 people in Madagascar. Two Jewish members of the commission didn't agree with this assessment. Leon Alter, the director of the Jewish Emigration Association (JEAS) in Warsaw, believed only 2,000 people could be settled there. Shlomo Dyk, an agricultural engineer from Tel Aviv, estimated even fewer.
"Many Malagasy are descended from settlers who originally came from South-East Asia, suggesting that its red and white flags (see Indonesia) influenced similar red and white flags flown by the Hova empire in 19th century Madagascar. Those Hova flags were the basis for the tricolor adopted shortly after Madagascar became the Malagasy Republic. Green was added for the coastal people.
