Lutheranism does not demand one system of church government. Some Lutheran churches in Europe have bishops and some have connections with the state. The Wisconsin Synod and other confessional Lutherans have a congregational system of church government in which congregations have control over their property and internal affairs though their voters assembly. All congregations and pastors must remain faithful to biblical teaching. If they do not, they cannot continue to be members of the larger synodical organization.

The geographic spread of Lutheranism in the United States was extended by migrations to the western frontier and by the large immigrations during the 19th and early 20th centuries of Germans, Norwegians, Swedes, Danes, and Finns. Many of these immigrants settled in the Midwest, and from there later pushed on to the far West. Since immigrants brought with them from Europe a variety of languages and customs, they organized in congregations and later synods according to their national origins. It was largely the prolongation of linguistic and ethnic barriers that prevented Lutheran union until well into the 20th century, when the barriers broke down and advance into intra-Lutheran ecumenical relations became rapid.

Biblical criticism had only a slight impact on nineteenth-century Lutheranism. Instead, Lutherans focused on confessionalism and predestination. Divisions arose between those who favored inclusive confederation (the General Synod), confessional subscription (the General Council and the United Synod, South), and complete unity in doctrine and practice (the Synodical Conference). The General Synod acquired a new appreciation for its Lutheran heritage in the last quarter of the nineteenth century and committed itself to the Augsburg Confession, but ... continued a good relationship with evangelical denominations and enacted no bar on altar or pulpit fellowship. Despite this, closer relations did develop between the General Synod, the General Council, and the United Synod, South, at the end of the century. During the 1870s, the Synodical Conference was itself divided over predestination (or the "election of grace").
