Lipid hydroperoxides are non-radical intermediates derived from unsaturated fatty acids, phospholipids, glycolipids, cholesterol esters and cholesterol itself. Their formation occur in enzymatic or non-enzymatic reactions involving activated chemical species known as "reactive oxygen species" (ROS) which are responsible for toxic effects in the body via various tissue damages. These ROS include among others hydroxyl radicals, lipid oxyl or peroxyl radicals, singlet oxygen, and peroxinitrite formed from nitrogen oxide (NO), all these groups of atoms behave as a unit and are now named "free radical". These chemical forms are defined as any species capable of independent existence that contains one or more unpaired electrons (those which occupy an atomic or molecular orbital by themselves). They are formed either by the loss of a single electron from a non-radical or by the gain of a single electron by a non-radical. They can easily be formed when a covalent bond is broken if one electron from each of the pair shared remains with each atom, this mechanism is known as homolytic fission.

Lipid molecules have these properties because they consist of mainly carbon. Some lipids are used for energy storage, others serve as structural components of cell membranes, and some are important hormones. Although the term lipid is often used as a synonym for fat, the latter is in fact a subgroup of lipids called triglycerides.

Lipid altering medications are used in lowering blood levels of undesirable lipids such as LDL cholesterol and triglycerides and increasing blood levels of desirable lipids such as HDL cholesterol. Several classes of medications are available in the United States, including HMG CoA reductase inhibitors (statins), nicotinic acid, fibric acid derivatives, and medications that decrease intestinal cholesterol absorption (bile acid sequestrants and cholesterol absorption inhibitors). Some of these medications are primarily useful in lowering LDL cholesterol, others in lowering triglycerides, and some in elevating HDL cholesterol.
