Even though Karnov may look like your everyday sidescroller, it is very unique. You have 2 hit points. When you take one hit, you turn blue, literally! After that, take another hit, and it's over for you! You can get to different power-ups, like being able to shoot more than one fireball and taking to the skies! This game can ... be funny at times.
In a way, Karnov was far ahead of it’s time – many games nowadays let you play as heroes of less than upstanding moral character. The game runs much slower than the arcade original, and doesn’t look nearly as pretty – but that’s a given, considering the limitations of the C64. The slower running is actually a benefit, though – the game is quite difficult, and dodging enemy fireballs, skeletons riding ostriches, Pillsbury doughboys throwing rocks, and flying green demons trapped up to their waist in brown, log-looking like objects (I don’t wanna know!) is made a little easier.

Karnov is a solid adaptation of the 1987 arcade game, matching the original's stage layout, graphical style and soundtrack within the limitations of the NES hardware. This version improves on its arcade predecessor by toning down the difficulty level through the removal of certain enemies, and allowing Karnov to be hit twice before dying. Still, the arcade game wasn't particularly impressive to begin with, and this version isn't different enough to make it anything more than average. ~ Skyler Miller, All Game Guide

Gameplay is very straightforward – and you have the option of just going through blasting everything Karnov hates, (Which is almost everything living) or by playing a bit more of a finesse game using your inventory. A decent commodore side-scroller all around, but a bit hard for some players.

Data East's four-way scrolling title starring Karnov, the fire-breathing Russian, who is on a quest to find the Lost Treasure of Babylon. He runs, swims, jumps, climbs and flies through nine levels of gameplay. Countless bizarre enemies of various sizes and strengths are out to prevent Karnov from finding the treasure.

The thing that separates Karnov from the aforementioned games is that it is incredibly, mind-numbingly hard. Just getting the hero through the first level of the game (much less all nine) is a challenge tough enough for even the most adept of gamers.
