Jack was born into a wealthy family, with aspirations of following in the footsteps of his surgeon father, who told Jack at an early age that he would never make a great leader. Following college and early in his career as a spinal surgeon, Jack operated on a young girl, in the process accidentally severing a nerve sac in her spine. Following a brief panic, he counted to five and fully repaired the damage, a story he would later recall to Kate as a way to manage fear. Jack later operates on Sarah, a woman involved in a car accident with Adam Rutherford (Shannon's father). Speaking to Sarah before the operation, he vows that despite the high likelihood of her becoming a paraplegic, he will "fix her." After the operation, he runs the stairs in a stadium to relieve stress, where he meets and races Desmond for the first time.

In "Groundhog Day," Jack is excited for his favorite holiday, Groundhog Day, and he can't wait to find out if Gertrude the Groundhog will see her shadow or not. But the scared groundhog runs into the clubhouse because she's too shy to face all of the people and cameras outside. Jack and the gang learn this is Gertrude's first Groundhog Day as the groundhog that everyone gathers around to see. Brunk Stinegrouber (JON STEWART) from the Groundhog News Network reports on whether Gertrude the Groundhog will face her fears and finally step out of the clubhouse. That night, Jack was visited in the Pearl's cellar by Bootstrap Bill Turner, currently serving as part of Davy Jones' crew aboard the Flying Dutchman. Turner informed Jack that his time was up, and Jones was calling in his debt. To reinforce this, Turner branded Jack with the Black Spot, setting fear into Jack's heart and causing him to begin his quest for the key with all haste, though he was so fearful of what his debt might entail, Jack could not bring himself even to tell his crew what forces pursued them.

The next day Jack, Kate and Charlie hiked into the jungle to find the cockpit. Once inside, they found the pilot alive. He gave them the transceiver shortly before being pulled out the window by the monster. Kate, Jack and Charlie ran from the cockpit and into the jungle. After a brief rainstorm, they found the pilot dead in a tree.

With the Sword and its scabbard in his possession, Jack was able to recite an incantation that unlocked its power—but ... caused the spirit of Hernán Cortés to materialize. Jack, wishing to use the Sword's full power, was unaware that Cortés, in teaching Jack how to correctly wield the Sword, was ensuring he, Cortés, would ultimately regain control of the weapon for his own dastardly use. Jack was sent to collect the eye of Stone-Eyed Sam, recently given to the merfolk, meaning Jack was forced to return to Isla Sirena. There, he fought two iguana monsters for his freedom, and obtained the eye from Tia Dalma. Dalma also reminded Jack that a dangerous foe was still pursuing him, a thought that Jack did not savour.

Jack's next challenge came on day 40, when Locke brought Boone to him, claiming he had fallen off a cliff. Jack wanted to amputate Boone's leg, and when Sun tried to stop him, he said, "Don't tell me what I can't do." He had a last minute change of heart when Boone awoke and told him he wanted Jack to let him go. Jack held Locke responsible for Boone's death and confronted him at the funeral. Even after Locke explained what had actually happened, his earlier lie and the the fact the he was keeping the Hatch a secret caused Jack to distrust him.
