Iron deficiency anemia can be associated with low dietary intake of iron, inadequate absorption of iron, or excessive blood loss [1,16,35]. Women of childbearing age, pregnant women, preterm and low birth weight infants, older infants and toddlers, and teenage girls are at greatest risk of developing iron deficiency anemia because they have the greatest need for iron [33]. Women with heavy menstrual losses can lose a significant amount of iron and are at considerable risk for iron deficiency [1,3]. Adult men and post-menopausal women lose very little iron, and have a low risk of iron deficiency.

Iron being a constituent part of the blood itself, there is a direct indication for the physician to prescribe it when the amount of haemoglobin in the blood is lowered or the red corpuscles are diminished. In certain forms of anaemia the administration of iron rapidly improves the blood in both respects. The exact method in which the prescribed iron acts is still a matter of dispute. Ralph Stockman points out that there are three chief theories as to the action of iron in anaemia. The first is based on the fact that the iron in the haemoglobin of the blood must be derived from the food, therefore iron medicinally administered is absorbed. The second theory is that there is no absorption of iron given by the mouth, but it acts as a local stimulant to the mucous membrane, and so improves anaemia by increasing the digestion of the food.
Iron is the only nutrient for which women have a higher daily requirement than men. The U.S. Recommended Daily Allowance (RDA) of iron for men is 10 milligrams for men and 15 milligrams for women.

It is estimated that infants can use greater than 50% of the iron in breast milk as compared to less than 12% of the iron in infant formula [1].
