Inspra from Canada Inspra contains eplerenone, a blocker of aldosterone binding at the mineralocorticoid receptor. INSPRA is indicated to improve survival of stable patients with left ventricular systolic dysfunction (ejection fraction 40%) and clinical evidence of congestive heart failure after an acute myocardial infarction. INSPRA tablets contain 25 mg or 50 mg of eplerenone. The principal risk of INSPRA is hyperkalemia. Hyperkalemia can cause serious, sometimes fatal, arrhythmias. This risk can be minimized by patient selection, avoidance of certain concomitant treatments, and monitoring.

Could Inspra stage a recovery? Pfizer could use another blockbuster, with some of its lesser drugs going generic. It has deep expertise in larger heart disease markets and touts hypertension drug Norvasc as the most prescribed high blood pressure medicine, while its cholesterol pill Lipitor is the world's most lucrative drug. Inspra is usually not used in patients with moderate to severe kidney problems. Medications such as spironolactone, triamterene and amiloride should be stopped prior to starting Inspra. Also, potassium supplements may need to be stopped to prevent increases in serum potassium levels.
