American Dental Plan, founded in 1978, is Arizona's oldest voluntary dental benefit plan. Yearly memberships start at only $54 per year. Members receive their initial exam and first 4 x-rays at no charge with additional dental services including cleanings, fillings, extractions, root canals, crowns and dentures, (and orthodontics!) available at 20-40% savings. In many instances, you will save enough money at your first dental visit to pay for your yearly membership!

Individual dental plans are available for online enrollment on a per-person basis only. The only payment option is by credit or debit card. Group dental quotes can be requested much the same way as group health quotes as discussed above.

A vision discount plan is included with your dental plan. The vision plan includes discounts on eye exams, eyeglasses, contact lenses and other prescription eyewear when provided by participating providers.
