Have a physician or clinic complete each required Immunization section that is required for the student and sign the form using black ink. (See What is Required ) Records from other medical offices clinics are acceptable and may be attached to the form. (See Where to Get Your Records)

The Immunization Record for International Students (below) indicates the required and recommended immunizations and tests you will need before you register for classes. Procure these in your home country, with the exception of the TB skin test, which must be done in Iowa.

Non-compliance warnings for students who do not respond to the early request for immunization data will appear in the registration system when they register for classes and again by email. Registration for the next semester or session will be prevented for students who do not provide this required data to the Office of the Registrar. In order to maintain the confidentiality of medical record information, immunization data provided to other University offices (e.g., Student Health Center) cannot be used to comply with this state mandated requirement.

Sue Swedo, of the National Institutes of Health, and Irva Hertz-Picciotto, of the MIND Institute, reported that major autism research databases would incorporate exposure histories - including immunization records. David Schwartz, director of the National Institute of Environmental Health Sciences (NIEHS), suggested that applications for developing exposure and body burden testing methods for autism be made to the NIEHS's Exposure Biology Programme, part of its Gene-Environment Initiative.
