Traditional Western, or Community Herbalists base their work on traditional folk medicine or indications of historical uses of herbs and modern scientific information. Backgrounds may include folk, Native American, eclectic, wise woman, earth-centered or other traditions. They may be trained through traditional or non-traditional methods such as apprenticeships, schools or self-study. Medical or Clinical Herbalists are present in the United States and in most of the nations in the European Union. Professional education is offered in the USA and throughout Europe in a variety of formats. Most programs cover the traditional uses of herbs, the basic medical sciences of biochemistry, nutrition and anatomy as well as diagnosis and prescription.

Japanese herbal medicine ... known as Kampo is part of the East Asian Chinese medicine tradition. Kampo is fundamentally a clinical system based on the classical medical literature dating back to the Han Dynasty in ancient China. In Japan today, fully 75% of physicians use at least some of the traditional Kampo formulas. In the United States, full strength Sho-saiko-to (H09) is available in granules or a lesser strength formula can be found in tablet form as Liver Kampo to consumers.

Because of the variety of herbs used in Chinese herbal medicine, there is a potential for negative interactions with prescribed drugs. Some herbal preparations contain other ingredients which are not always identified. The FDA has issued a statement warning diabetics to avoid several specific brands of Chinese herbal products because they illegally contain the prescription diabetes drugs glyburide and phenformin. FDA warnings have been issued for PC-SPES and production of that product stopped because the Chinese herbal products ... contained prescription drugs (indomethicin, diethylstilbestrol and warfarin).
