Heartburn is described as a sour, burning sensation in the upper abdomen or chest. It may go up into the neck and mouth and be associated with acid regurgitation or food "sticking" in the throat. The burning, pressure, or pain can last as long as two hours and is generally worse after meals, while laying down at night, or while bending over.

Heartburn occurs when digestive juices from the stomach move back up into the esophagus, the tube connecting the throat to the stomach. The upper third of the esophagus consists of skeletal muscle that propels the food downward. The lower two-thirds of the esophagus is smooth muscle. The lower esophageal sphincter (LES) is a thick band of muscle that encircles the esophagus just above the uppermost part of the stomach. This sphincter is usually tightly closed—opening only when food passes from the esophagus into the stomach—and prevents the contents of the stomach from moving back into the delicate esophageal tissue. The stomach has a thick mucous coating that protects it from the strong hydrochloric acid it secretes to digest food.

Heartburn is the most common symptom of a condition known as gastroesophageal reflux, or acid reflux, or GERD. It usually begins as a burning pain that starts behind the breastbone and radiates upward to the neck. The stomach walls are made to withstand the acids that are produced by digesting foods. The esophagus, which is the tube that passes food from the throat to the stomach, has a very thin lining to it. This lining does not have the protective strength of the stomach intestines. Thus when stomach heartburn acid reflux into the esophagus it makes the lower section of the esophagus painful and irritated. This painful inflammation is called reflux esophagitis.
