Tension headaches usually affect both sides of the head and usually appear at the front of the head, although they can appear at the top or back of the skull. Tension headaches often begin in the afternoon and can last for several hours. They can occur every day. When this happens it is called a chronic tension headache.

Most headaches are tension headaches, caused by a muscle spasm in the back of the head and neck. The spasm can be sparked by emotional stress or by holding the head in a fixed position (for example, while facing a computer screen or driving for hours). Sometimes the pain can be very severe and felt in the back of the head and encircling the head in a vise-like band.

The pain associated with rebound headaches is a pressing, dull, diffuse pain that is typically felt all over the head (much like tension-type). Rebound headaches are usually worse in the morning hour and often can be felt on the front or top of the head. Typically, a cycle of medication use, followed by partial headache relief, followed by another headache, can be suggestive to "rebound / medication-induced headaches".
Many people have 1 or 2 headaches of this type every month. Sometimes they develop more frequently, typically during times of stress. Approximately 3% of people have a tension-type headache on most days. When this happens, the headaches are called chronic tension-type headaches.
Acute tension headaches not associated with a disease are treated with muscle relaxants and analgesics like aspirin and acetaminophen. Stronger analgesics, such as propoxyphene and codeine, are sometimes prescribed. As prolonged use of these drugs can lead to dependence, patients taking them should have periodic medical checkups and follow their physician's instructions carefully.
