Genasense, Genta's lead anticancer drug, is a novel targeted therapy that blocks the production of Bcl-2, a protein that appears to be a fundamental cause of resistance to cancer treatment. By knocking down Bcl-2 in cancer cells, Genasense may enhance the effectiveness of chemotherapy in patients with advanced melanoma. In its initial trial, Genta performed the largest randomized controlled trial that has ever been conducted in patients with advanced melanoma. A summary of the long-term follow up data from that study, on which the design of the new trial is based, appears below.

Skeptics have another interpretation: Genta and Aventis are getting the sense that Genasense doesn't work and that they are powering up their studies to increase the chances of approval. Sheer size could get the drug approved but Warrell says his firm has not seen any results. He calls speculation that Genasense doesn't work based on the trial-size increase "insane." The chances of approval are improved because it has been about a quarter of a century since new treatments for advanced melanoma have reached the market.

Genta deliberately diversified its program, he said, so if Genasense failed in one area, there would be multiple chances of getting approval in another. Now, the company is relying on its fallback strategy.
