Gallbladder: Cholecystectomies

There are two surgical options for removing the gallbladder: the "open" procedure and videosurgery. The videosurgical technique is referred to as laparoscopic cholecystectomy, and it is the way most people have their gallbladder removed today.
The diagnosis of gallbladder carcinoma is usually unsuspected. Early-stage carcinoma is typically diagnosed incidentally because of inflammatory symptoms related to coexistent cholelithiasis or cholecystitis. One percent of patients undergoing cholecystectomy for cholelithiasis has an incidental gallbladder carcinoma (30).
Physicians in Operating Room... Some patients with very serious complications from gallbladder disease may not be eligible for laparoscopic gallbladder removal. In addition, patients in the third trimester of pregnancy should not usually undergo laparoscopic cholecystectomy, because of risk of damage to the uterus during the procedure.

Because of the high incidence of gallbladder cancer in a calcified (porcelain) gallbladder, patients with this finding should be strongly considered for an open cholecystectomy even if they are asymptomatic. It is best to avoid a laparoscopic cholecystectomy in this setting to avoid the risk of peritoneal seeding if, indeed, gallbladder cancer is present.

The standard cholecystectomy is a major abdominal surgery in which the surgeon removes the gallbladder through a 5- to 8-inch incision. Patients may remain in the hospital about a week and may require several additional weeks to recover at home.
