Fosamax, a drug prescribed to treat osteoporosis, may cause a condition called osteonecrosis of the jawbone. Fosamax (alendronate), a drug that has been on the market for almost 10 years, has been linked to osteonecrosis of the jaw (ONJ). Otherwise known as "dead jaw", osteonecrosis is a severe condition in which the jaw dies and is unable to regenerate, eventually leading to extreme pain and exposed bone in the mouth. Fosamax is part of a class of drugs known as bisphosphonates, which are used to treat osteoporosis and bone pain caused by some types of cancer. Bisphosphonates are often used in the management of certain cancers—such as lung, breast, prostate, and multiple myeloma—where cancer has metastasized (spread) to the bone. Bisphosphonates are absorbed into the bone tissue where the drug may remain for some period of time. It is believed that the drugs interfere with the deposition and absorption of calcium in the bone possibility leading to the degeneration of the bone tissue, and may apply to both oral and intravenous administration of the drug.

If you take Fosamax every day and you miss a dose, skip that dose and take the next regularly scheduled dose the following day. Missing one dose will not affect your treatment. Do not take two tablets at the same time. If you take a Fosamax once a week and you miss a dose, take the missed dose on the morning after you remember. Do not take two tablets on the same day. Return to taking one tablet once a week, as originally scheduled on your chosen day.
For Fosamax to be effective, you must take the tablets without food or other medications, exactly as directed. Fosamax is effective only when each tablet or bottle of oral solution is taken with a full glass of plain water first thing in the morning, at least 30 minutes before the first food, beverage, or other medication. If you can wait longer before eating or drinking, the medication will be absorbed better. Do not lie down after taking Fosamax until at least 30 minutes have passed and you've had something to eat.
