Pelvic laparoscopy The cause of endometriosis is unknown. However, there are a number of theories. One suggests that the endometrial cells (loosened during menstruation) may "back up" through the fallopian tubes into the pelvis, where they implant and grow in the pelvic or abdominal cavities. This is called retrograde-menstruation.

One possible cause of endometriosis is that during menstruation, menstrual tissue backs up into the fallopian tubes, enters the abdominal cavity, and implants. Most physicians believe that all women experience some type of back up at some point, but women who have immune problems go on to develop endometriosis.
This is the most widely accepted theory for the cause of endometriosis. During "retrograde menstruation," a woman's menstrual flow backs up through her fallopian tubes, implants in her abdomen, and grows. Some experts believe that all women experience retrograde menstruation, but that women with endometriosis may have either a hormonal or an immune system problem, and as a result, the implants are allowed to grow.
