Endometriosis is a disorder of the female reproductive system. The uterus is composed of 3 layers: the perometrium (the outer layer), the myometrium (the muscle or middle layer), and the endometrium (the inner layer).

A competing theory suggests that endometriosis does not represent transplanted endometrium but starts de novo from local stem cells. This process has been referred to as coelomic metaplasia. Triggers of various kind (including menses, toxins, or immune factors) may be necessary to start this process.

Because the abnormal growths associated with endometriosis are made up of the same kind of tissue and blood vessels found in the uterine lining, any endometrial implants will act just like the endometrium in the uterus. That means they respond in the same way to the hormonal changes of the menstrual cycle.

Because oestrogen is known to stimulate the growth of endometriosis, hormonal therapy has been designed to suppress oestrogen synthesis, thereby inducing atrophy of ectopic endometrial implants or interrupting the cycle of stimulation and bleeding. Implants of endometriosis react to gonadal steroid hormones in a manner similar but not identical to normally stimulated eutopic endometrium.
