The objectives of surgical treatment of endometriosis is to restore normal anatomy, to excise or destroy all visible disease, and to prevent or delay recurrence of disease. Surgery for the treatment of endometriosis can be performed via laparotomy or laparoscopy. Laparoscopy offers the advantages of better visualization, less tissue trauma and desiccation, smaller incisions, and a shorter postoperative recovery. Deeply infiltrating endometriosis involving the rectovaginal septum requires extensive surgery.

Treating endometriosis with medication has definite limitations. Medication usually controls mild or moderate pain and may eliminate small patches of the disease. But large endometrial cysts in the ovary are less likely to respond, and drugs cannot remove scar tissue. Surgery to remove adhesions, implants, or endometriomas may be needed to relieve pain or improve fertility. Even with surgery, all endometriosis may not be eradicated and sometimes postoperative medical therapy is used.

Investigators are studying certain chemicals detected in blood tests that may prove to help diagnose endometriosis and so avoid invasive diagnostic procedures in many women. Among the most studied to date are CA-125 and CA19-9. Both are elevated in women with severe endometriosis. Higher levels of both chemicals occur in many other diseases... including ovarian cancer, so results using this test alone do not provide enough information for a definitive diagnosis of endometriosis.
