Air in the bladder in the presence of a nearby segment of diverticulitis is suggestive of a colovesical fistula. If a colovesical fistula is suspected, the administration of rectal contrast material may be helpful. In this case, intravenous contrast agent should not be used so that the presence of contrast enhancement in the bladder can allow definitive diagnosis of a fistulous connection. Alternatively, a CT cystogram may be obtained with the intravesical administration of contrast material via a Foley catheter without the use of an oral or rectal contrast agent.

Diagnosis of diverticulitis is usually based on clinical history and physical examination. X rays are taken to exclude other causes of abdominal pain like appendicitis, abscess in the abdomen or inflamed colon, which may mimic an acute diverticulitis. A CT scan of the abdomen is helpful in atypical cases. A colonoscopy (lower GI scope test) is generally not advisable during an acute episode.

Only 15 to 20 percent of people who have diverticulosis develop diverticulitis. The major factors associated with diverticulitis include advancing age and lack of dietary fiber. In many cases people can help prevent this disease simply by including more high-fiber foods in their diet.
