On April 1, 1999, a Diablo II Screenshot of the Week featured cows fighting. People wondered if the screenshot was an April Fool's joke or if there really was a Secret Cow Level planned for Diablo II. It turned out that there was a cow level. To access the level, one must kill Diablo (or, in Lord of Destruction, kill Baal), return to Rogue Encampment in Act I within the same difficulty level, and then transmute Wirt's Leg with a Tome of Town Portal in the Horadric Cube. This will open a portal to the secret level. There is an item set named "The Cow King's Leathers" which may only be collected in this secret level.
In Diablo II, players return to a dark world plagued by evil forces. After possessing the body of the hero who defeated him, Diablo resumes his nefarious scheme to shackle humanity into unholy slavery by summoning the powers of the other Prime Evils, Mephisto and Baal. As one of five heroes, players face a new series of quests to rid the world of Diablo and his vile brethren forever.

Popular pastimes in Diablo II include Baal runs and Cow runs. Only Paladins can kill Baal and they are blessed with so much AI that they have the impressive ability to kill him repeatedly without a player controlling them, creating portals for other players to leech XP from them as they use their hammers of l33t pwnage. Many players will spend up to 15 hours at a time running with Baal, who is a big fan of jogging and healthy eating, explaining his infinite HP.
