Cystoscopy under anesthesia with bladder hydrodistention under pressure to about 80-100 cm water pressure is a recognized treatment for interstitial cystitis. Many patients report significant, but often transient, symptomatic improvement with bladder hydrodistention alone. In about 20% of patients, symptoms may improve for several months. Because of the limited response, some practitioners no longer consider hydrodistention an efficacious therapy for interstitial cystitis. Cystoscopy with hydrodistention may be considered for first-line therapy in patients with more severe symptoms.

Your doctor may prescribe an oral medication, pentosan (Elmiron), the only oral drug approved by the Food and Drug Administration specifically for interstitial cystitis. How it works is unknown, but it may restore the inner surface of the bladder, which protects the bladder wall from substances in urine that could irritate it. It may take two to four months before you begin to feel pain relief and up to six months to experience a decrease in urinary frequency. Side effects include minor gastrointestinal disturbances and possible hair loss, which reverses when you stop taking the drug. Not for use by pregnant women, Elmiron may cause bleeding and loss of pregnancy. Make sure your doctor knows if you're pregnant.

Interstitial cystitis (IC) appears to be a relatively new disorder. First reports of IC emerged during the early 20th century, but it did not become a subject of medical investigation until the number of cases markedly increased during the 1970s. IC is almost exclusively a disorder suffered by women, who are estimated to make up 90% or more of the total cases. The number of people affected by IC is unknown; it has been suggested that most cases still go undiagnosed, but there are about 100,000 diagnosed cases at any given time (precise statistics are not available), and it has been claimed that this may be only 20% of the actual cases. The median age of symptom onset is 40, with only 25% of patients under the age of 30, most of those in their late 20s.

ELMIRON is indicated for the relief of bladder pain or discomfort associated with interstitial cystitis. While the mechanism of action is not fully understood, ELMIRON is thought to adhere to and help reinforce the bladder's protective layer and may act as a buffer, preventing irritants from reaching the bladder wall. A scientific study showed that ELMIRON relieved pain/discomfort in 61 percent of IC patients who continued ELMIRON treatment for three months. Additional patients reported relief of pain/discomfort by six months. Relief is gradual. The clinical value and risks of continued treatment in patients whose pain has not improved by six months is not known.

Interstitial cystitis is a chronic, painful inflammatory condition of the bladder wall characterized by pressure and pain above the pubic area along with increased frequency and urgency of urination. This occurs because of chronic inflammation of the lining of the bladder and swelling of the interior walls of the bladder. Affected individuals urinate frequently with pain even though there is no diagnosed bladder infection.
