The safety agency says that while it was negotiating its 1996 settlement, Cosco was collecting reports of children becoming trapped in the easily misassembled metal crib. Cosco did report that an 8-month-old boy died in the crib, but the safety agency says the firm didn't report when an 11-month-old died in a crib mattress incident. Cosco says it didn't know of the death.

Cosco knew about 10 incidents of the mattress sold with its cribs compressing allowing infants to become entrapped in the mattress platform. An 11-month-old in Joliet, Ill., died when he fell feet first through the slats of the mattress platform and became entrapped at the neck. Cosco knew about incidents occurring with the crib at the same time it was in talks with the CPSC to settle allegations from an earlier civil penalty case. Cosco paid a $725,000 fine in 1996 for not reporting incidents associated with its toddler beds.

Child-seat maker Cosco sells at the rate of about 1 million a year a type of booster seat that has been linked to deaths and serious injuries in car crashes and crash tests. All other manufacturers have stopped selling booster seats with shields in the USA amid mounting evidence that they do not protect well in crashes and as parents are being urged to keep children between 40 and 80 pounds in boosters. The National Highway Traffic Safety Administration and the American Academy of Pediatrics have warned parents against using shield boosters for years. NHTSA expected hard-to-meet 1995 standards using crash-test dummies the size of a 6-year-old child would prompt firms to stop selling shield boosters, say people involved with the agency's move. The seats use a padded shield instead of safety belts or harnesses and don't restrain the neck and shoulders.

Cosco learned of children being injured in its Two Ways Tandem Strollers, but did not report this information to CPSC. In one case, the stroller with two children inside collapsed in the path of an oncoming car and was nearly hit. After a complaint from a retailer, Cosco added a secondary locking mechanism to all strollers in its inventory. Cosco ... offered consumers, "upon request," a fix consisting of a secondary locking mechanism. When asked by the CPSC, Cosco reported it had received 3,000 complaints of locks failing, including 250 reports of strollers collapsing.

The ship's Chinese crew is still aboard, said Mike Hudson, a spokesman for Regal Stone Ltd., owners of the Cosco Busan. The ship, which is 902 feet long, is registered in Hong Kong.
