Citadel recognizes that developing an effective partnership with the hospital is paramount in ensuring long term solutions for increasing cash collections. The Citadel is the massive structure from which the Combine govern the Earth. One is located in City 17, and resembles a massive, metallic and largely featureless skyscraper, rising several kilometers into the sky from ground level, and extending some distance underground as well. The Citadel is heavily mechanized, and entire sections of it can move to allow for mass deployment of airborne Combine units such as scanners or hunter-choppers. It appears to either provide power to City 17 or drain power from it, as power lines can be seen extending out of it. These power lines then join on to regular transmission towers and poles, connecting it to the rest of the city.

The asari were the first race to discover the Citadel soon after learning to use the mass relays. They were joined shortly after by the salarians, then the volus. As other races were contacted by the asari or discovered the Citadel in turn, they gained embassies on the Citadel and came under the jurisdiction of the growing Council, which eventually became comprised of the asari, the salarians, and the turians.

[I]n the Ring Room of the Citadel, Truth was readying to activate the Halos, and one of his Brute Chieftains was torturing the captive Johnson. When SPARTAN-117 radioed that he was too far from Truth to prevent him from activating the Halos, Commander Miranda Keyes boldly took a Pelican Dropship and crashed it into the Citadel, landing in the midst of Truth, his attendant Brutes, and Johnson. Keyes opened fire on the Brutes, killing several before she realized that the situation was hopeless: she could not engage dozens of Brutes, then kill Truth, and then rescue Johnson. She aimed her Magnum at Johnson, offering to kill him in a coup de grace, until Truth rose and shot her several times in the back with a Spiker, killing Keyes.
