Ciprofloxacin: Drug Administration

Ciprofloxacin (Cipro®, Bayer), is one of the medications approved by the Food and Drug Administration for use after exposure to inhaled anthrax. It is among the top selling drugs in the United States and is used to treat a variety of other infections including urinary tract infections, skin infections, and pneumonia.

Ciprofloxacin is a faint to light yellow crystalline powder with a molecular weight of 331.34. It is soluble in dilute (0.1N) hydrochloric acid and is practically insoluble in water and ethanol. Ciprofloxacin Injection solution is available as a sterile 1% aqueous concentrate, intended for dilution prior to administration. The formula contains lactic acid as a solubilizing agent and hydrochloric acid for pH adjustment. The pH range for the 1% aqueous concentrate in vials is 3.3 to 3.9.

Ciprofloxacin, though generally well tolerated, has been associated with peripheral neuropathy and tendon rupture and, in 2004, the US Food and Drug Administration approved additional warnings about these effects for both ciprofloxacin and moxifloxacin. A warning about the risk of pseudomembranous colitis was added to the intravenous and long acting forms of ciprofloxacin.

Ciprofloxacin interacts with anti-asthmatic drug theophylline (... a component of caffeinated drinks) and can cause respiratory failure, seizures and cardiac arrest. Caffeinated drinks should be avoided. Patients should avoid taking ciprofloxacin with antacids.

Ciprofloxacin should not be combined with the drug theophylline, as serious or fatal reactions can result. It may interact adversely with phenytoin and warfarin. Ciprofloxacin may be less effective when used in combination with vitamin supplements containing zinc or iron or with antacids containing calcium, aluminum, or magnesium.
