The frequency of allopurinol-induced hypersensitivity reactions may be increased in patients with decreased renal function who receive allopurinol and a thiazide diuretic concomitantly. Allopurinol should usually not be administered to patients who have previously shown hypersensitivity to it or who have had a serious reaction to the drug.
Allopurinol is a structural isomer of hypoxanthine (a naturally occurring purine in the body) and acts to inhibit xanthine oxidase. This enzyme is responsible for the successive oxidation of hypoxanthine and xanthine resulting in the production of uric acid, the product of human purine metabolism.[1] In addition to blocking uric acid production, inhibition of xanthine oxidase causes an increase in hypoxanthine and xanthine, which are converted to closely related purine ribotides adenosine and guanosine monophosphates. Increased levels of these ribotides causes feedback inhibition of amidophosphoribosyl transferase, the first and rate-limiting enzyme of purine biosynthesis. Allopurinol therefore decreases both uric acid formation and purine synthesis.

Allopurinol can decrease breakdown of azathioprine (Imuran), mercaptopurine (6-MP), cyclosporine (Neoral, Sandimmune) and theophylline (Theo-Dur, Theolair, Theo- 24) by the liver, increasing blood levels and side effects. Doses of azathioprine and mercaptopurine need to be reduced when they are used together with allopurinol. Mercaptopurine can be substituted for thioguanine (6-TG) to avoid this interaction altogether.

Allopurinol is used for the therapy of canine leishmaniasis in Europe and it is currently the main treatment for this disease in Israel. Allopurinol treatment is oral, inexpensive and can be prescribed by the attending veterinarian and administered by the owners of the dog at home. This study was designed to assess the clinical and parasitological efficacy of allopurinol in dogs with visceral leishmaniasis (VL). Five dogs with naturally-occurring VL were treated with 20 mg/kg of allopurinol once daily for 8 months. The dogs were physically examined and tested pre- and during treatment. Testing included: the determination of serum anti-leishmanial antibody titers by ELISA, recording of globulin levels and albumin/globulin ratio, evaluation of the parasitological status by culture of splene and lymph node aspirates, xenodiagnosis and the determination of the infectivity to Lutzomyia longipalpis sand flies.
