Aldactone (spironolactone) is a mild diuretic, manufactured widely throughout the world. Medically this class of drug is used to treat high blood pressure, efficiently lowering the retention of water and salt. Aldactone acts by reducing the amount of aldosterone secreted by the adrenal gland, which is the hormone primarily responsible for water regulation in the body. This effect is beneficial to competitive bodybuilders, who need to shed subcutaneous water before a showing. Specifically this compound is a potassium-sparing diuretic, much weaker in effect than both Dyazide and Lasix. As can be surmised, potassium levels are not greatly reduced with Aldactone.

Aldactone was used almost exclusively during the last week before a competition. Since this causes neither a dramatic nor an immediately noticeable draining effect, spironolactone is usually taken over 5-6 days in a dosage of 2 tablets of 50 mg daily. Aldactone (spironolactone/hydrochlorthiazide) should not be used to expediently drain water at the last minute. Both male and female athletes take Aldactone. The side effects of potassium-saving diuretics are relatively low compared to thiazides and furosemides. The main problems in men who take this substance consist of gynecomastia and possible impotence.

Aldactone is indicated in pregnancy when edema is due to pathologic causes just as it is in the absence of pregnancy (... see PRECAUTIONS: Pregnancy). Dependent edema in pregnancy, resulting from restriction of venous return by the expanded uterus, is properly treated through elevation of the lower extremities and use of support hose; use of diuretics to lower intravascular volume in this case is unsupported and unnecessary. There is hypervolemia during normal pregnancy which is not harmful to either the fetus or the mother (in the absence of cardiovascular disease), but which is associated with edema, including generalized edema, in the majority of pregnant women. If this edema produces discomfort, increased recumbency will often provide relief. In rare instances, this edema may cause extreme discomfort which is not relieved by rest. In these cases, a short course of diuretics may provide relief and may be appropriate.
