Advair 250/50 is currently indicated for the maintenance treatment of airflow obstruction in patients with COPD associated with chronic bronchitis. Advair does not replace fast-acting inhalers to treat sudden symptoms. Lower respiratory tract infections, including pneumonia, have been reported following the inhaled administration of corticosteroids, including fluticasone propionate and Advair Diskus. Patients with COPD often have multiple risk factors for reduced bone mineral density. Advair Diskus may increase this risk, therefore, bone mineral density assessment is recommended prior to starting Advair Diskus and periodically thereafter. Long-term use of inhaled corticosteroids, including Advair Diskus, may increase the risk for cataracts or glaucoma.

Advair Diskus... known as "salmeterol xinafoate inhalation powder," was approved by the Food and Drug Administration ("FDA") in 2000 for the long term treatment of asthma and chronic obstructive pulmonary disease (COPD). Together with the asthma medication Serevent, Advair belongs to a class of drugs known as long-acting beta agonists, or LABAs. Advair, which combines Serevent with a steroid, had worldwide sales of $4.4 billion last year, making the medicine the biggest-selling product for the London-based GlaxoSmithKline.

"The availability of Advair in a metered-dose inhaler is good news for asthma patients," said Robert A. Nathan, MD, Clinical Professor of Medicine at the University of Colorado Health Sciences Center and a lead study investigator for Advair HFA. "It lets those who need an alternative delivery method to the dry-powder inhalers achieve a comparable level of effectiveness to that of Advair Diskus, but in a device that may be more appropriate for their needs."

The lower-strength Advair Diskus was approved in 2003 for maintenance treatment of airflow obstruction in COPD patients. The FDA questioned how the higher-strength Advair compared with the lower-strength version in order to allow for appropriate dosing recommendations, Glaxo said. Glaxo has sought the expanded approval of the higher-strength medicine for COPD, including chronic bronchitis and emphysema.
